President Donald Trump signed an executive order on Monday dramatically reducing the Centers for Disease Control and Prevention's routine childhood vaccine schedule from 18 to 11 diseases, splitting the MMR shot into three separate doses and ending the birth-dose hepatitis B vaccine. It's a sweeping overhaul that shifts medical decision-making power from federal agencies to parents. Flanked by Health Secretary Robert F. Kennedy Jr. at the White House, Trump directed the Department of Health and Human Services' Task Force on Safer Childhood Vaccines to report within 90 days on implementation, urged states to align school vaccine laws with the new recommendations, and ordered the Justice Department to investigate states violating parental and religious exemption rights.
A schedule out of step with peer nations
The executive order recognizes what the administration calls "gold standard childhood vaccine recommendations for only 11 core vaccinations against the most serious and dangerous diseases, along with the MMR, which hopefully will be split up," Trump said during the signing ceremony.
The order draws on an HHS study concluding that the prior 18-disease schedule required more shots than comparable wealthy nations across Europe and Asia demand of their own children — "more childhood vaccines than any peer nation, and even twice as many doses as some European countries," Trump said.
The administration's push went further, with the CDC previously accepting a separate scientific assessment — examining 20 peer developed nations — that found the U.S. is a "global outlier" in vaccine count and does not achieve higher vaccination rates than countries recommending fewer shots, according to a decision memorandum signed by Deputy HHS Secretary Jim O'Neill, the CDC's acting director.
"By eliminating the birth Hep B and the new RSV jab from the 'recommended' schedule, vaccination is delayed to 2 months of age, rather than the first day of life," said Brian Hooker, Ph.D., chief scientific officer at Children's Health Defense. "Infant deaths in the U.S. peak during the first week of life and anything that can be done to address that will help drive mortality rates lower."
Spacing out shots and expanding parental control
The order calls for parents to have five separate vaccine appointments instead of children receiving multiple shots at once. "I think you're going to see some amazing results [by] breaking it up," Trump said. "It's not just that you're doing fewer vaccines or jabs, as they say, but you're doing them in a series of visits to the doctor. Much smaller amounts."
The order explicitly frames vaccine decisions as parental rights rather than government mandates. "Parents can still choose to give their children all of the vaccinations if they wish, it's up to the parents — we're making recommendations that are strong — but it's up to the parents if they wish and they will be covered by insurance," Trump said.
The memo urges parents to "make individualized decisions for higher-risk children through shared clinical decision-making," and directs the Justice Department to "advance legal challenges against states that violate children's rights to religious or medical vaccination exemptions," Trump said.
A step toward accountability, if not full transparency
After years of Americans watching pharmaceutical companies reap record profits from a one-size-fits-all vaccine schedule pushed with little room for individual medical judgment, the shift toward parental choice and informed consent is a welcome course correction. FDA Commissioner Dr. Marty Makary said: "Public health works only when people trust it." That trust was badly eroded by pandemic-era mandates that punished families for declining shots rather than earning their confidence through transparency — and this order, however incomplete, is a step toward rebuilding it.
